1. After having successfully completed your Online Application for Admission in SLU, accomplish the
needed information above using black ink. Please write legibly.
2. Give this form to your Senior High School Principal or his/her designated authority together with a
white long envelope.
3. Bring the sealed envelope addressed to the School Dean you are applying for in SLU on your scheduled
test schedule and submit it to the University Registrar’s Office personnel. For accuracy, please refer to
the list of Dean of Schools in SLU.
4. All submitted sealed envelopes with all information provided will be used by SLU in evaluating your
application. Hence, it will not be made available to you.
