THE PURPOSE OF THE USER'S SATISFACTION SURVEY IS TO KNOW THE OPINION OF CUSTOMERS TO START FROM THIS INFORMATION TO IMPLEMENT ACTIONS THAT ALLOW TO IMPROVE THE QUALITY IN THE PROVISION OF
THE SERVICES OFFERED AND THE BIOSEGURITY RULES IMPLEMENTED.
Thank you for conducting the user satisfaction survey. It will not take more than five minutes to complete it and it will be of great help to improve our services. The data contained therein will be
treated anonymously.

1 = BAD 2 = REGULAR 3 = ACCEPTABLE 4 = GOOD 5 = VERY GOOD

5. ¿Are personal protection elements used for the worker and for the user used? (Gloves, mask, uniform, protectors, among others)
6. ¿Are hand washing procedures frequently promoted and implemented in workers and users?
7. ¿Are cleaning, disinfection and sterilization procedures carried out frequently and adequately?
8. ¿Is the separation and disposal of waste (disposable items and others) in appropriate containers and bags promoted and implemented?
9. ¿Is there clear and relevant information about healthy living habits in the establishment?
10. ¿Is any preventive measure implemented for the application of pharmaceutical products (correct use, sensitivity test, allergic reaction management)?
