Management Certification on Incident Reporting

I hereby certify that the Department of General Services (DGS) has reported all incidents and
“near-misses” which have come to my knowledge during the period of 01 July 2018 to 31 December
2018 in accordance with the guidelines under the Bank-wide Protocol for Incident Escalation, as
amended under MB Res No. 1956 dated 25 November 2015.

This certification is being issued in accordance with the requirement for a

semestral Management Certification in the abovementioned Bank-wide Protocol for Incident Escalation.
