Dr. Elliot Fuqua, MD

Family Practitioner
Frankfort Regional Medical Center
234 St. Malinaw City

Dear Mr. Fuqua,

Madeline Ashton has assisted my surgical team for a little over a year and her performance has
been nothing short of outstanding. In my experience as a podiatrist at Englewood Hospital and
Medical Center, I have seen many nurses come and go. but very few as hard working and
devoted as Ms Ashton; she is a first-rate nurse.

Ms. Ashton is extremely professional, punctual, studious, and astute. While I’m operating she
often knows what instruments and tools I need before I even ask. During post-surgical patient
care, Ms. Ashton is sensitive to the needs of patients and their families, and she’s able to plan,
implement, and evaluate treatment of the patient with efficiency.

I recommend Ms. Ashton without reservation for the nursing position in your surgical
department. Her organizational skills and professionalism have been a great asset to this
hospital and she will no doubt continue to excel in any nursing position she obtains in the
future.

Sincerely,

Lilia Mendez

Janine Wallace
Manager of Sales
Healthy Consulting Agency
49938 Leopard Rd.
Georgetown, WA 73652

Dear Ms. Wallace,

I would like to offer my sincerest apologies for my misconduct. I was unaware that my jokes
and comments were so offensive to my fellow female colleagues. I understand that my
inappropriate behavior has caused problems in the office, and I plan to do everything in my
power to amend the situation.

First, I plan to apologize to each and every person that I offended. I also have signed up for a
sexual harassment seminar so that I can better understand the proper workplace behavior. I
feel that taking this seminar will help me to change my manners, helping me to be a better co-
worker. I also will make sure to never act in the same manner again.

I know that in order for our team to work efficiently, everyone must work together and feel
comfortable with each other. My misconduct interrupted this flow, hurting the company’s
sales. Through making my amends individually and working hard to change my conduct so that
it will never happen again, I hope to heal what I broke. I have great faith in the company, and I
do not want my actions to jeopardize the great work we do. Again, I apologize profusely for my
misconduct, and I promise it will not happen again.

Sincerely,

Paul Brown
