PROMISSORY NOTE

I, _________________________________, days after date, for value received, promise to pay 4HG Multipurpose Cooperative (MPC) the sum of

I hereby authorize __________ to deduct ______________________Pesos (P__________) from my monthly salary every payroll to beg in on
_____________, 20___ and until the loan is fully paid.

I hereby agree that should I resign or be terminated from my employment with __________, the outstanding balance of my loan s hall be paid through: (1)
deduction from share capital; (2) value of collateral (if any); (3) by my co-maker; (4) last claim.

In case my co-maker resigns, I will be resposible to find another co-maker within 24 hours after he/she tendered his/her resignation, otherwise, my
outstanding loan balance is due and demandable.

CO-MAKERS OATH
I,_______________________________, a regular member of 4HG Multipurpose Cooperative voluntarily and without any hesitation nor constraints
whatsoever, agree to be the co-maker of the borrower, Mr/Ms. ___________________________, and guarantee the loan he/she is availing.
In case the borrower resigns from the cooperative and he/she has not processed his/her last claims, I shall pay the borrower’s outstanding obligation
after deducting his/her share capital once he/she fails to pay his/her financial obligation from the cooperative when all collection efforts has failed.

With this, I hereby authorize __________ to deduct ______________________Pesos (P__________) from my monthly salary every payroll to begin on
_____________, 20___ and until the outstanding loan of the resigned borrower is fully paid.
Furthermore, I hereby permit the Coop to withhold the release of my share capital contribution until such time the principal borrower has found my
replacement as co-maker.
Upon my resignation, i hereby authorize _________ and _________ to offset the borrower's outstanding obligation from my share capital contribution
and last claims.
