All insurance MUST be showing extended to LTA as beneficiary; MUST be extended
to LTA, else will reject.
4 . Site Photo to show Construction Access/Occupation of lane etc.
- Please indicate clearly all relevant distances from junctions/bus bay/accesses in
the photo.
- For lane closure, please provide lane closure plan view and photos with indicate
total length closure at one time.
And state the date and time for the lane closure period (Max. 21 days per
application)
5 Location Plan showing roads name and entrance location.
6 . Detail Site Utilisation Plan with site access, method statement for traffic
control, traffic control plan (traffic sign) if occupation of lane.
7. Section view if crossing over PUB drain, drain size (depth and width), crossing
MUST be 300mm away and 100mm higher than the drain.
8. Method statement of work for lane closure.
9. Completion date for the project (Duration of the project)
* please avoid entrance location with only 1 lane with central island
* please avoid entrance within 35m from bus stop, and 50m away from the Road
Junction. LTA will reject most likely.
* the commencement date for the work applied must be at least 14days after the
submission date.
* daily charge applied to the lane closure.
