AFFIDAVIT OF NON-EMPLOYMENT

I, MARIA DE LA TORRE, 26 years of age, married. Filipino Citizen and a resident of Brgy. Abay,
Maasin, Iloilo, Philippines, after having been duly sworn to in accordance with law, do hereby
depose and state:

1. That to this date, I am not employed in any capacity at any corporation, company or
individual person or entity, or with the government agency or institution;

2. That I am a full-time housewife managing our family home and taking care our domestic
animals at Brgy. Abay, Maasin, Iloilo;

3. I am executing this Affidavit of Non-employment to attest to the truth of the foregoing

and to support my application for Correction of Clerical Error for RA 10172 (GENDER) in
my Certificate of Live Birth before the Office of the Municipal Local Civil Registrar of
Maasin, Iloilo;

4. I am executing this Affidavit to attest to the truth of the foregoing statements and for
whatever legal purposes and intents this may serve.
