THIS IS AN AGREEMENT FOR PROVIDING MEDICINES FREE OF COST FOR PATIENTS: BETWEEN
NARAYAN MEDICAL STORE AND PRANAAM ENT AND EYE CARE

1. Duration

From 10-05-2019 until terminated by either party (agreement to be reviewed annually)

2. Service objectives

The objective of this Agreement is to secure the provision and availability of Medicines free of
cost for the patients treated under Ayushman Bharat Yojna and other empanelled schemes by
Pranaam Ent And Eye Care

Service to be coverd

The main elements of the service to be covered by this agreement are

 Provision of medicines free of cost for Indoor patients and at the time of discharge .

3. Charging

This is agreed that the charges of the abovementioned medicines will be collected by Narayan
Medical Store from Pranaam Ent And Eye Care at reasonable prices.

DECLARATION : I Mr Akhilesh Gupta hereby state that I am the Proprietor of Narayan Medical
Stores which is Certified with Licence no: CG- RZ4-36082 dated 2-5-2019.

A copy of form 20 & 21 is attached with this agreement.
