Proof of permanent address(Voter ID card, Passport,Driving License,and Aadhaar Card) as declared in the application form with the name of police station and District. Muslim OBC and Muslim ST candidates will be required to submit the respective Category Certificate issued by the competent authority recently. For candidates who have applied in PWD category, certificate for disability is required from a Government Certified Authority. Candidates seeking admission under Kashmir Migrants category will be required to submit a certificate in support of their claim of being Kashmiri Migrants obtained from registered Divisional Commissioner, of the concerned State. Candidates seeking admission under candidates from Jammu & Kashmir category will be required to submit a state-domicile certificate in support of their claim for being from Jammu & Kashmir. Character Certificate from the institution last attended. Certificate from an MBBS doctor or a Government hospital certifying blood group. In case an intervening/gap period is Involved, the candidate has to submit a certificate/affidavit from class 1 Gazetted Officer/Notary Public for the entire intervening/Gap period showing year-wise pre-occupation and non indulgence in any criminal activities after leaving the institution last attended. Affidavit for not indulging in ragging activity as per prescribed Performa, In case of programs for which the eligibility requirement is graduation(B.A/B.SC/B.COM etc), 2 candidate whose result of the qualifying examination has not been declared, may be considered for provisional admission subject to. Submission of certificate issued by the concerned college/University to the effect that he/she has ‘appeared in the final semester / year of the concerned qualifying examination. Il. Verification of Transcripts/Marks Sheets for the period preceding the final semester/year for the qualifying examination to establish the minimum eligibility requirements of the program that he/she is clear in all the papers preceding final semester/year.
