PROJECT PROPOSAL
Title: ICT INSTRUCTIONAL MATERIAL(TELEVISION)

Rationale: Pursuant to DepEd Order # 54, s. 2009 entitled “Revised Guidelines Governing Parent
Teachers Associations (PTAs) at the school Level.
1. A PTA is authorized to collect voluntary contributions from parents/guardian-members.
2. Based on our school constitution and by laws article 1 No. 2 that parents support
immediate needs of pupils.
3. It is the aim of this project to help the government in ways that the school can and involve
parents, teachers, and pupils. With this, facilities of schools would be improved through
Voluntary donations to support the institution financially. Specifically, the project aim to
provide ICT instructional material(32 inches television) to improve the teaching and
learning process through power point presentations or video presentations.

Objectives: General: HPTA Voluntary donation for the ICT

Specific: 1. Help the school raise finances for various purposes such as
improving facilities.
2. Support teachers and learners in the ICT instructional aid.

Beneficiaries: The 38 pupils, 5 school personnel, and HPTA

Planning Committee: Homeroom Parent Teachers Association

MEMORANDUM OF AGREEMENT

We the Homeroom PTA officers and members had agreed during our meeting last July 24
and July 30, 2018 that parents/guardians of Grade IV- Dahlia shall voluntarily contribute two
hundred fifty pesos(P250) for ICT Instructional material for the teaching and learning process.
As basis to move on with the purchase of the project was approved by parents during the
Grade IV HPTA meeting on July 24, 2018 held at Grade IV classroom.
We do affix our signature to support the HRPTA Project to support the immediate need of
our children.
