NATIONALREFERENCE LABORATORY FOR CLINICAL CHEMISTRY

Dear Participants:

Pursuant to the Department Circular No. 2017-0199 of the Department of Health “Agreement
on the New Timelines for the Application and Issuances of Certificates of
Performance/Certificates of Proficiency of External Quality Assessment Program
(EQAP) conducted by the National Reference Laboratories”, the LCP-NRL-CC is pleased
to inform all Secondary and Tertiary Clinical Laboratories that we are ready to accept
registration for the 2019 National External Quality Assessment Scheme for Clinical Chemistry
(NEQAS-CC) starting April 1 to May 31, 2019.

The participation fee of Php 8,000.00 is payable either in cash or cheque (Government or
Manager’s cheque) only. Payment can be made thru the LCP Cashier or you may deposit in
any LANDBANK branch using the following details.

Other modes of payment will not be accepted.

As required, completely fill up the attached registration form (preferably typewritten) and submit
all mandatory documents to LCP-NRL-CC office on or before May 31, 2019. All payments
made after the said deadline will no longer be accepted.

Subsequently, a Certificate of Performance, instead of Certificate of Participation, will be

issued as we conclude the 2018 NEQAS-CC Program Cycle this coming June.
