Dear Madam,

Please except our apology’s for the inconvenience and personnel frustration you have
experienced as a result of the processing delay of your refund claim.

As you know, we have rules and procedures to folllow in the identity determination and
refund payment to claimants. Sometimes; however, these rules, which are intended too
provide protection for the consumer may seem bureaucratic and unbending.

However, it is extreme important that refund claims be delivered


to the proper owners identity verification and the provision of proof of purchase are claimant
responsibilities.

We would like to continue in the processing of your claim, however we must again ask you
for a a provision of proof of former residence at the address in Dallas. Once this address
verification is recieved, our evaluation will be continue.

A lack of response from you within sixty days will be taken as an indication of your lack of
interest in the pursuit of your claim and your file will be closed accordingly.
