RADIOLOGIST'S REPORT

Service:
Clinical information: right thigh swelling – 3 months,
progressive increasing in size, non mobile, no skin changes .
with on and off fever .

MRI RIGHT THIGH


Sequences: Coronal T1/T1Tirm/T1FS, Sagittal T2/T1FS and
Axial T1/T1FS/T2FS.
Post gadolinium: Ax/Coronal/Sag : T1FS, Coronal T1 .

Findings
There is a large lobulated mass measures 11.4cm (w) x 8.0cm
(AP) x 12.8cm (CC) in the posterior compartment of thigh,
between the femur and the biceps femoris and
semitendinosus. Mass extends laterally and medially, and in
close proximity to cortex of the right femur, sparing the only a
small anterior portion, no cortical destruction. There is
heterogenous enhancement of the mass with IV gadolinium.
There is intramedullary enhancing component lobulated 2.0cm
(AP) x 6.0cm (CC) in bone marrow of the distal diaphysis femur.
Mass appears predominantly isointense in T1 with areas of T1
hyperintensity, as compared to muscles, and heterogenous in
T2/T1tirm with
peri-tumoral edema. No obvious fatty component.
There is an intramuscular enhancing component measures
1.3cm x 0.9cm in vastus medialis muscle. Interfascial fluid
collection superomedially measures 5.0cm (w) x 8.0cm (CC).
Fluid collection and oedema in the subcutaneous tissues
laterally.

COMMENT
Large tumour in the posterior compartment right lower thigh
with intramedullary (bone marrow) component in the lower
right femur, and intramuscular component in vastus medialis
muscle. Possibilities includes sarcoma (chondrosarcoma,
ewing's sarcoma) or PNET (primitive neuroectodermal tumour).
Suggest biopsy.
