CORRECTIONAL OFFICER

At approximately 08:00 AM on 30/09/2019

I was carrying out my duties as TRAINEE CORRECTIONAL OFFICER when the

following incident occurred: (Insert post/role description)

At approximately 0800Hrs on the 22nd of August 2019 I was on Parade at the Brush Farm
Corrective Services Academy Eastwood when this incident occurred.
Before forming up on for Parade I realised that I had left my Correctional Officer Baseball
Cap at home. I notified Trainee Correctional Officer Joshua Dorrans who was acting as Duty
Student for the day that I did not have my Cap wiuth me and that I would be out of uniform
for Parade. I formed up without my cap and after Parade had finished I spoke with our
Instructor Mr. Scott Westlake and informed him that I had left my cap at home. He reiterated
the uniform requirents for all Trainne Correctional Officers at Brush Farm and that I was in
breach of those requirements and was out of uniform on Parade. I was asked by Mr.
Westlake to submit an Incident Report on this event to account for the missing piece of
uniform and continued with my activities for the day. At 5pm today I confirmed that the
absent Cap was left on my kitchen bench at my residence at 31 Messina Crescent
Bonnyrigg Heights NSW 2177 and has now been accounted for.
I submit this report to the Governer for their information.
