TREASURER’S AFFIDAVIT

I, Jenelien O. Dominguez, being duly sworn, depose and say:

That I have been elected as Treasurer by the stockholders of JUB CREDIT

COOPERATIVE, to act as such until my successor has been duly elected and qualified
in accordance with the by-laws of the cooperative, and that as such Treasurer;

I have been authorized to receive for the cooperative all subscriptions paid in by
the subscribers for the capital worth of THREE HUNDRED THOUSAND PESOS (Php
300,000) ; and

That the total membership fees paid and actual received by me is ONE
THOUSAND FIVE HUNDRED PESOS (Php 1,500).

IN WITNESS WHEREOF, I have hereunto affixed my signature this 18 th day of

September, 2019 in Cagayan de Oro City, Philippines.
