AFFIDAVIT

I, Abdukadil A. Barahim, Filipino, of legal age, married and a resident of Mercedes,

Zamboanga City, Philippines, after being sworn to in accordance with law, depose and
state:

1. I was formerly employed under Nemesis Security Agency;

2. Nemesis Security Agency pays our Philhealth contributions which are deducted
from our salary;

3. On or about 22 October 2018, the latter’s contract with Universidad de

Zamboanga expired and was never renewed;

4. On that same day, Universidad de Zamboanga entered into a new contract with a
different security agency, which is Aloha Security Agency;

5. We were not relieved from our duty as security guards but we were absorbed by
Aloha Security Agency;

6. Aloha Security Agency is now our employer and the agency that pays our
Philhealth contributions, which is also withheld from our salaries;

7. That I am executing this Affidavit to attest to the truth of the forgoing and to
comply with the requirements of Philhealth in claiming our Philhealth I.Ds and
for whatever legal purpose this may serve.
