We, the Board of Directors of La Terraza Villas Homeowners


Association, do hereby certify that at the Board Meeting held last September
29, 2019, the following resolution, was approved:

RESOLVED, as it is hereby resolved, that the


Board of Directors authorizes
________________, ___________ of La Terraza
Villas Homeowners Association Inc.
(LTVHOA) to transact with the bureau all tax
matters including but not limited to
application for official receipts, printing,
report, etc.

The purpose of our request is to be able to issue


official receipts to homeowner paying their
homeowner monthly dues.

This application for issuance of receipts is


initiated after all efforts to retrieve previous
issued receipts from previous officers failed.
Hence, considered lost.
