I MARY JOY O, AMORA, Filipino, of legal age married and with residence business address at
block 6 lot 15 Dahlia St. Villa Luisa Subdn. Phase 2 Brgy San Agustin, Dasmarinas, Cavit, after having
duly sworn according to law, depose and state THAT:

1. I am the registered owner of business under the trade name: KRIZZANDREI’S SARI SARI
STORE Located at Brgy. San Agustin 111 Dasmarinas City Cavite.

2. The Above business was duly registered at the Department of Trade and Industry [ DTI] on the
Day of June 5, 2015and was issued Certificate # 05395781 and BIR Certificate of
Registration # OCN IRCOO1147505 Dated August 22, 2018

3. That at present I am Causing Cancellation due to :

A) Continues losing of my business operation.
B) Stiff rivalry of business in our place of which I cannot sustain to finance the on going losses of
my business.
C) I cannot hired a trusted sales personnel to attend my day to day sales operation.

4. That as far as my personal knowledge as registered owner, I attest to the fact that KRIZZANDREI’
SARI-SARI STORE has no standing Financial Obligation to any individual public or private
corporation or entities.
5. That I am executing this affidavit to attest the truthfulness of the foregoing declaration penalties of
perjury to respectfully request to Department of Trade and Industry to Cancel Certificate of
Registration #05395781 moreover to serve all legal intents.
