AFFIDAVIT

WE, SYLVIA C BAQUIRAN and MELANIE C BAQUIRAN, both Filipino citizens, all
of of legal age, single, with postal address at No. 4-A Cypress Village, Masambong,
Quezon City, after having been duly sworn in accordance with law, hereby depose and
say:

That we are the registered owners of a parcel of land covered by TCT no.
RT-67175 located at Quezon City.

That said title was mortgaged to TULAY SA PAG UNLAD INC under PE No. 1149 inscribed
on August 3,1989 in the amount of FIFTY THOUSAND PESOS (P50,000.00)

That the said loan was paid and cancellation of mortgage was released and annotated at the back
of the said TCT under PE no. 1489 was inadvertently inscribed 1489 instead of 1149,

That the said title was not mortgaged to any person or entity.

That the discrepancy was noticed when the title is about to transfer by the buyer.

That we are requesting for the correction of the said mortgage under PE no. 1149 for its cancellation.
