AFFIDAVIT OF SERVICE

I, ARIAN MARIE B. FAJARDO, of legal age, Filipino and with office address at
Unit 119 Sun Mall, Sun Residences, Welcome Rotonda, Sta. Teresita, Quezon City, after
being duly sworn, hereby depose and say:

1. That I am the Property Consultant of FAST, ORGANIZED AND RELIABLE

MODES OF SERVICE (FORMS) CORPORATION with same address;

2. That today October 7, 2016, upon the instructions of GILBERT RONDILLA,

Unit Owner of Unit 3858 of Tower 2, Sun Residences, I served copies of the
Letter of Demand dated September 28, 2016 (which is attached as Annex
“A”) upon MS. ROSITA M. HAYASHI, the tenant of said unit through the
following:

a. Dropping a copy on the mailbox of Unit 3858 on October 7, 2016.

b. By sending a copy thereof on Ms. Hayashi’s email address:
rosemanrique52@gmail.com on September 28, 2016 at 11:20 in the
morning. (In fact, Ms. Hayashi replied on the same email message
which October 4, 2016 at 8;52 in the evening. Copy of said email
message is attached herewith as Annex “B”)

3. That I am executing this affidavit to attest to the truthfulness of the foregoing

facts and for whatever legal purposes it may serve.
