I, MELIZZA ELUMBRA MANJERON, of legal age, single, and a resident

of 154-E Sinciego Street, Brgy 18, Zone 2, Pasay City, after having been duly
sworn on oath in accordance with law, do hereby depose and say:

That I am the accused in Criminal case No. CC 9421-2019 for Regional

Trial Court, TARLAC;

That I posted a cash bond amounting to Ten Thousand Pesos (10,000.00)

with the Clerk of Court , RTC Branch 63, Tarlac City, for my provisional liberty;
That I hereby bind myself to appear and present at all stage of trial of the
above –entitled case when duly notified, failing which, the court may declare me
to have waived my rights to be present during the trial and that the trial in
absentia may proceed.

That I have executed this Affidavit for the information of the proper
authorities and for whatever legal purpose it may serve;
