affordable education. Entrance examinations are
administered as scheduled from September to
April of each school year. Applicants shall be
informed of the scheduled date and time of the
examination upon their application at the
Admissions OAce.

Admission Guidelines
#
Application Requirements:
1. Completely Dlled-out Application form.You

2. 2 pcs. ID picture (plain background), size 1½”
x 1½”
3. Photocopy of any of the following indicative
of the student’s strand:
a. Grade 12 Report Card
b. CertiDcate of Good Moral Character (with
school year)
c. CertiDcate of Enrollment
d. Current school ID with school year and
year level
4. Application fee (if applicable)
5. Additional Requirements:
a. For applicants from schools in the
Philippines or schools abroad under
DepEd, who graduated in the previous
years (graduation year is 2017 and prior)
and did not enroll in any
college/university after graduation:
i. Original and photocopy of Grade 12
Report Card
ii. CertiDcation from the high school
principal regarding the student’s
F137-A.
Contents of the certiDcation are as
follows:
a. The student’s Form 137-A is still
in the school Dle
b. The student’s F137-A has not
been forwarded to another
college/university
iii. Additional requirements for foreign
nationals:
a. Alien CertiDcate of Registration
(ACR) or Special Study Permit
(SSP)
b. Original and photocopy of
Passport
b. For graduates of Non-Formal Education
Accreditation and Equivalency or
graduates of Alternative Learning System
Accreditation and Equivalency:
i. CertiDcate of Rating (with passing
marks in all subjects)
c. For graduates of Associate Degree
Programs:
i. Transcript of Records
a. No failing grade
b. Weighted average of at least 2.00
or its equivalent, excluding
Religion, PE and NSTP
ii. Honorable Dismissal/CertiDcate of
Transfer Credential
iii. Photocopy of NSO Birth CertiDcate
iv. Photocopy of the CertiDcate of Good
Moral Character
d. For applicants coming from schools
abroad (except for Filipino schools under
DepEd):
i. Scholastic Records duly
authenticated by the Philippine
Foreign Service Post located in the
student applicant’s country of origin
or legal residence
ii. CertiDcate of completion of
secondary education (senior high
school / grade 12 or its equivalent)
iii. CertiDcate of Good Moral Character
or Recommendation from the School
Guidance Counselor or Principal
iv. For Filipino applicants:
a. Birth certiDcate printed on
National Statistics OAce (NSO)
paper
b. Original and photocopy of Filipino
passport
v. For foreign nationals:
a. Alien CertiDcate of Registration
(ACR) or Special Study Permit
(SSP)
b. Original and photocopy of data
page of the applicant’s passport
showing date and place of birth,
and birth certiDcate or its
equivalent duly authenticated by
the Philippine Foreign Service
Post.
c. Five (5) copies of the Student’s
Personal History (PHS) duly
signed by the applicant, both in
English, and in his/her national
alphabet accompanied by his/her
personal seal, if any, and
containing, among others, his left
and right thumbprints and a 2x2
inch photograph on plain white
background taken not more than
six months prior to submission of
his/her documents.
d. A notarized aAdavit of support
including bank statements or
notarized notice of grant for
institutional scholars to cover
expenses for the student’s
accommodation and
subsistence, as well as school
dues and other incidental
expenses.
NOTE:

* The application is considered oAcial once a


receipt has been issued:
MIT’s oAcial receipt (for those who
applied in-campus) or
MIT’s provisional receipt (for those who
applied at any accredited Testing Center)
* Examinees should present their
examination permits together with the
receipts on the day of the examination.

Enrollment requirements in
accordance with CHED’s Manual
of Regulations for Private
Higher education (MORPHE):
1. Mapúa Examination Permit or Examination
Result Letter or Notice of Results
2. Original grade 12 report card (Form 138) with
the applicant's eligibility for admission to
college duly signed by the school principal
3. Fully accomplished Student Data Sheet /
Undergraduate Enrolment Form
4. Original birth certiDcate printed on PSA / NSO
paper
5. Original certiDcate of good moral character
(dated not earlier than February of the
graduation year) from the high school (with
school seal)
6. 1 pc. ID picture (plain background), size 1½” x
1½”
7. One (1) long brown envelope to contain all of
the above
8. Additional requirements:
a. For graduates from schools in the
Philippines or schools abroad under
DepEd, who graduated prior to the current
school year and did not enrol in any
college/university after graduation:
i. CertiDcation from the high school
principal regarding the student’s
F137-A.

from senior high school in the
Philippines or schools abroad under

Additional Requirements:
1. For enrollees beginning 2nd or 3rd term:
OAcial Results of Basic Medical Laboratory
Work-up (CBC, Urinalysis, Chest X-Ray: PA
View) from any major hospitals.
2. For Valedictorians and Salutatorians (of duly
accredited Philippine high schools with at
least 60 graduating students): a certiDcation
of honors indicating the total number of
graduates signed by the high school principal
(with school seal).
