PROMISSORY NOTE

In consideration with the case for Qualified Theft now pending

before the Regional Trial Court - Branch 21, I, ________________ of legal
age, Filipino, and a resident of _______________, promise to pay
___________________ or to its ORDER, the sum of ________________________

schedule below without need of notice or demand.

MATURITY

Payment of the principal amount shall paid in cash and in an

installment basis for a period of two years with the following maturity
dates:

1st payment not later than December 15, 2017 ____________

2nd payment not later than March 2018 ____________
3rd payment not later than June 2018 ____________
3rd payment not later than September 2018 ____________
Final payment not later than December 2018 ____________

CONDITIONS

This note may be paid ahead of the scheduled dates by the

Borrower at any time, in whole or in part.

If the Borrower fails to make the payment as stated in the schedule,

the remaining outstanding balance of the principal sum shall forthwith
become due and payable at the election of the holder/s of this note.

The Borrower will incur 6.0% monthly interest of the unpaid principal
amount in case of delay.

PENALTY CLAUSE

If it should be necessary to bring suit to enforce payment of this

note, the maker/borrower shall pay, in addition to the aggregate
principal sum plus legal interest from the date of such delay, a sum
equivalent to TWENTY FIVE (25%) percent of the total amount then due
thereon to the holder/s as attorney’s fees.

Maker/Borrower waives presentment, demand, notice of demand,

protest, notice of protest or notice of nonpayment in connection with the
delivery, acceptance, performance, default or enforcement of this note
or of any documents or instrument evidencing any security for payment of
this note.
