How to Fill in your

Self-Certificate Form
It is mandatory that you fill in and submit the self-certificate form when you are unable to attend
classes due to illness or any other reason.

This is extremely important as it is not only ours but also your responsibility to keep track of your
class attendance which will have great impact on your final result review.

Please follow the instructions below:

1. Download the Self-Certificate Form from Moodle

2. Save the Self-Certificate Form to your H Drive

3. Complete the form, make sure all required information are filled in
Note: Full description for Reason for Absence must be provided – short sentence e.g. ‘sick’
or ‘illness’ will not be accepted.

4. Name your file in the following order:

[your 8 digit student ID] [full name] [Self Certificate] [date.month.year].dox
e.g. 40001234 April Tsai Self Certificate 20.7.2015

5. Submit the form by emailing it to your home tutor and/or Programme Leader
Note: DO NOT print out and submit the form in paper!

6. Take care and get well soon! You will be missed on class.
