Type of Incident Forest Fire Date of Incident April 27, 2019

Location of Incident Canduag near Settling Pond Area Time of Incident 02:00 PM

Initial Description of Incident:

On April 27, 2019 at around 02:00 PM. Forest fire was spotted near Canduag Settling Pond. Safety
Department immediately coordinated to Environmental office regarding the incident. WT-19-12 and
RCDC water trucks immediately responded to the fire incident, and around 03:00 PM additional four (4)
Water Trucks responded, WT-19-13, WT-11, WT-10 and WT-08. As per initial assessment and
investigation, the fire started at the center of the said area. BT-04 hauling geotextile was spotted near
the area where fire incident happened. Forest fire was controlled around 06:30 PM. Until 09:00 PM Envi.
& Safety Dept. personnel and water trucks continued monitoring the area.

Immediate Actions Taken:

 Reported the Fire Incident
 Water Trucks and Envi. & Safety Dept. personnel responded to the incident
 Conduct initial investigation
 Continue monitoring
