FOLLOW THE FORMAT OF THE RESPECTIVE ORGANIZATION, INSTITUTION, OR COMPANY WHERE YOU PLAN TO PRESENT THE CONCEPT. GENERALLY, THESE SPECIFICATIONS MUST BE FOLLOWED: • Title of the Activity/Concept: All Caps and Bold • Font: Tahoma, Arial, or Times New Roman • Font size: 12 • Margins: 0.5” (Top, Bottom, Left, Right) • Spacing: Single • Bond Paper Size: 8.5” x 13” (long) • Page Numbers: Bottom Right of the Paper 2. THE DATE OF IMPLEMENTATION MUST BE REFLECTED. 3. THE RATIONALE MUST BE COMPOSED OF AT LEAST TWO PARAGRAPHS AND MUST NOT EXCEED 200 WORDS. IT SHOULD REFLECT AN OVERVIEW OF THE ACTIVITY AND PROVIDE A JUSTIFICATION FOR ITS CONDUCT. 4. THE OBJECTIVES MUST BE GENERALLY AND SPECIFICALLY STATED. IT MUST ALSO BE ALIGNED WITH THE VMGO OF THE ORGANIZATION, INSTITUTION, OR COMPANY. 5. THE ACTIVITY GUIDELINES SHALL BE COMPOSED OF THE PRE-PLANNING STAGE, IMPLEMENTATION STAGE, AND EVALUATION STAGE AND SHALL BE STATED IN NARRATIVE FORM. THREE STAGES OF ACTIVITY IMPLEMENTATION • Pre-planning stage shall include the dates and procedures of pre-conferences/meetings with the core group, working committees and consultation with focal persons and administration. • Implementation stage shall contain the guidelines, steps, procedures as to the actual presentation of the activity. • Evaluation stage shall include the dates and procedures of post conferences/ meetings with core group, working committees and consultation with focal persons and administration. 6. THE BUDGETARY REQUIREMENTS SHALL BE PRECISELY ITEMIZED WITH 10% ALLOWANCE FOR EMERGENCY EXPENSES. IT MUST BE PRESENTED IN MATRIX FORM. 7. PROVIDE A COVER LETTER, THE LOGISTICS, AND AN APPROVAL SHEET. 8. SUBMISSION OF THE CONCEPT PAPER MUST BE A MONTH PRIOR TO THE DATE OF IMPLEMENTATION TO GIVE ENOUGH TIME FOR THE ADMINISTRATORS TO REVIEW AND FOR THE PROPONENTS/ ORGANIZERS TO PREPARE FOR THE ACTIVITY.
