Orthopedics Rotation Tips, lovingly written by Alexander Chang, PGY - 2
First day/setting up:
Miss Leigh Underwood is a staff member at Southern bone and joint and doesn’t directly work under Dr. Barbieri, you’d contact her (she prefers phone call, 601- 554-7400, leave a message if she doesn’t pick up) at least 1 week prior to starting your rotation to set up a timing for you to sign a bunch of paperwork to allow you to work in Southern Bone and Joint, the Surgical Center and Orthopedic institute (aka “the OI”). Southern Bone and Joint, the Surgical Center AND OI buildings are all connected. Southern Bone and Joint 3688 Veterans Memorial Drive Hattiesburg, Mississippi 39401 At times you’d be required to assist in surgeries Dr. Barbieri has over at Forrest General Hospital. You pretty much don’t see Miss Underwood after the first day. Her office is on the 2nd floor of Southern Bone and Joint. Dr. Barbieri’s office is located on the 3rd floor, look for the office located around the “G - #” section of the 3rd floor after you meet Miss Underwood. She doesn’t know anything about a Forrest Badge, you’d need to get it from Human Resources of Forrest General Hospital itself. Call Tory Evans (male) at 601-288-1207 before or within the first week of your rotation. You’d set up an appointment with him to get your picture taken and a badge made. OSS Building-1st floor, 125 S. 28th Ave There’s still this weird rivalry between Forrest and Wesley, so don’t wear your Wesley badge even if you haven’t gotten your Forrest badge in Forrest or the OI. Forrest General Hospital: At times, you’d have to drive over to Forrest for surgeries, this is located on the 2 nd floor, Same-day surgery would be close by the Operating rooms. You’d usually be in the Operating rooms rather than the Same-day surgery suites. I’d suggest parking on the 2nd floor of the parking ramp for easy access to the OR floor. If on call, you’d be on site with Dr. Barbieri all day, you leave when he leaves. Schedule: Monday – Clinic starts at 0730 hours usually goes to 1630 hours, sometimes 1730 hours. He may have surgeries after clinic ends on Mondays. Tuesday – Clinic starts at 0730 hours usually goes to 1630 hours, sometimes 1730 hours. Wednesday – Go to normal EM Didactics, then text Dr. Barbieri after to see where to meet him/what to do. Thursday – Clinic starts at 0730 hours usually goes to 1630 hours, sometimes 1730 hours. Friday – Clinic starts at 0730 hours. He may have surgeries after clinic ends on Fridays (Surgery overflow day). Weekends – You’re off usually, unless Dr. Barbieri is on call, then you’d be with him ALL-day err’day. Usually he’s on for 2 weekends per month (sometimes it might be one day every week, depending on month). **Call schedule: If he’s on call on a normal clinic day, you’d be at clinic as per normal, however, if he’s called for an emergent consult/surgery, you’d go with him to see the case and/or scrub in to assist. Patient logs/procedure logs: Surgeries – You’d ask the nurses (or just grab one from the chart) for a sticker for documentation for each case in the OR. You’d need it to log your patients. Clinic – Miss Evelyn (Dr. Barbieri’s nurse/secretary who has worked for him for 13 odd years) can give you a print out of each day’s clinic schedule. Ask her for it each clinic day so you have a patient log. You’d be doing a LOT of steroid injections in clinic. Make sure you log these. Things to review prior to rotation: X-rays: Dr. Barbieri will be viewing a LOT of X-rays throughout each day, be familiar with the basic anatomy, if you have questions, just ask. MRIs, CT scans: On ER we know some basics of CT, but MRIs would be quite alien, try and read up on some basics of MRI imaging prior to starting. Human Anatomy: Review the musculature, nervous innervations of the body. Especially of the hand. Flexor compartment of the forearm, extensor compartment of the forearm, musculature and nerves of the hand, fingers, etc. After all, he is a Hand specialist, you’d be seeing a lot of upper extremity cases. If you don’t know things, ask, he is very willing to teach. It’s good to have reviewed the above basics, however if you do not know, he will teach it to you. “There is no such thing as a stupid question, don’t hesitate to ask” – Dr. Barbieri. Miscellaneous: Dr. Barbieri enjoys talking about cars, sports at/for leisure. If you’re a sports and car fanatic, you’re set. Food – Sometimes the nurses or staff bring in food to the surgery lounge to share. As poor residents it’s key to making use of this. Coffee is available there too. There used to be food provided to residents in the OI cafeteria free of charge, sadly this went away, Dr. Barbieri sometimes will purchase you a meal. There’s a physician/staff lounge in the Surgical Center and OI as well as FGH (lounge in FGF is the largest but usually bereft of actual food). Attire – You’d be in dress clothes and white coat all clinic days (urgh bummer, I know), but Dr. Barbieri believes that OR scrubs should only be in the OR. He didn’t say anything about scrubs to clinic (He usually dresses up in shirt and tie). Scrubs – There are scrubEx machines in FGH, the Surgical Center and OI. It’s recommended to change into a new set whenever you get in to the OR setting from the outside. However, a tip is just to get a few sets from Wesley and bring it with you. All the places use the same shade of green. You won’t have any credits for the scrubEx machines, however, the nurses are usually very willing to bring one out for you, just remember to give it back to them so they get their credits back (Every employee there only get 2 credits, sorry, you don’t you little 2nd year resident!). Dr. Barbieri doesn’t like to see people wear scrubs into work and wear the same scrubs back out in public, so be sure to change out. I’d suggest bringing some green scrubs from Wesley to change into when you get to FGH or OI.
