Restraint
There may be occasions when your attempts to de-escalate are unsuccessful and
consequently a decision is made to physically intervene to prevent the risks associated
with challenging behaviour. It should be emphasised that physical restraint of
consumers is an intervention of last resort and should be carried out only by health
professionals trained in safely facilitating this (Lamont et al. 2012). You should always
consider whether any alternative strategies are available, and if so, have these been
exhausted? Also, what would happen if you did nothing? These questions may be asked
in the context of alleged assault when considering whether reasonable force was
applied, either in a consumer’s best interests or as a basis for self-defence. Restraint
carries with it significant risks of injury to consumers and staff, and in some cases even
death (Boumans et al. 2014).
Box 24.1 Guiding principles for safe restraint (Health Education and Training Institute
2014; South Eastern Sydney Illawarra Mental Health and Illawarra Institute for Mental
Health 2010)
1. Restraint is the option of last resort to manage challenging behaviour; it is to be used
when other less coercive interventions are unsuccessful or inappropriate.
2. Any restriction to a consumer’s liberty and interference with their rights, dignity and
self-respect should be kept to a minimum and should cease as soon as the consumer has
regained self-control.
3. Restraint and seclusion should never be used as a method of punishment. All actions
undertaken by staff must be justifiable and proportional to the patient’s behaviour, with
the least amount of force necessary.
4. Staff must exercise reasonable care and skill to ensure the safety, comfort and
humane treatment of consumers in restraint or seclusion.
5. Communication and engagement should be maintained with the consumer at all
times, with all opportunities taken to deescalate the situation.
6. Pain compliance should never be used when restraining someone and any direct
pressure on the neck, abdomen, thorax, back or joints is to be avoided.
7. The consumer’s physical condition should be continuously monitored, with any
deterioration, in particular to the airway, noted and managed promptly.
8. All restraints should have an appointed leader throughout the restraint to maintain
safety.
9. Face-up restraint (supine) should be used where it is safe to do so. Face-down
restraint (prone) should be used only if it is the safest way to protect the consumer and
staff. Prone restraint should be used for only the minimum amount of time necessary to
administer medication and/or move the person to a safer environment.
10. Post-restraint debrief for the consumer, staff and any relevant others should be
undertaken in all situations.

Penerapan prosedur restrain berisiko menyebabkan cedera yang signifikan pada klien
dan staf serta dalam beberapa kasus bahkan kematian (Boumans et al. 2014). Ketika
upaya menurunkan perilaku non-adaptif tidak berhasil dan diputuskan melakukan
intervensi fisik untuk mencegah risiko terkait perilaku agresif perlu ditekankan bahwa
restrain fisik adalah intervensi terakhir dan harus dilakukan hanya oleh profesional
kesehatan yang terlatih dalam melakukan prosedur tersebut (Lamont et al. 2012). Harus
selalu dipertimbangkan apakah ada strategi alternatif.
Prinsip-prinsip panduan restrain secara aman (Institut Pendidikan dan Pelatihan
Kesehatan 2014; South East Sydney Sydney Illawarra Kesehatan Mental dan Institut
Illawarra untuk Kesehatan Mental 2010)
1. Restrain adalah pilihan terakhir untuk mengatasi perilaku non-adaptif dan
digunakan ketika intervensi yang kurang memaksa tidak berhasil atau tidak
sesuai.
2. Pembatasan terhadap kebebasan pasien dan gangguan terhadap hak, martabat
dan harga diri pasien harus dijaga seminimal mungkin dan harus dihentikan
segera setelah pasien dapat mengendalikan dirinya.
3. Restrain atau fiksasi tidak boleh digunakan sebagai metode hukuman. Semua
tindakan yang dilakukan oleh perawat harus dapat dibenarkan dan proporsional
dengan perilaku pasien.
4. Perawat harus melakukan perawatan dan tindakan yang beralasan untuk
memastikan keselamatan, kenyamanan dan perlakuan manusiawi kepada pasien
dalam restrain.
5. Komunikasi dengan klien harus selalu dijaga.
6. Tidak boleh menggunaan rasa sakit (pain compliance) ketika melakukan restrain
dan hindari tekanan langsung pada leher, perut, dada, punggung atau sendi.
7. Kondisi fisik konsumen harus terus dipantau, dengan setiap kemunduran,
khususnya jalan napas, dicatat dan ditangani segera.
8. Semua restrain harus memiliki pemimpin yang ditunjuk untuk menjaga
keamanan.
9. Restrain terlentang (supinasi) harus digunakan di tempat yang aman untuk
melakukannya. Restrain tengkurap (pronasi) harus digunakan hanya jika itu
adalah cara paling aman untuk melindungi pasien dan perawat. Posisi pronasi
harus digunakan hanya dalam waktu singkat seperti diperlukan untuk
memberikan obat atau memindahkan pasien ke lingkungan yang lebih aman.
10. Pembekalan pasca restrain untuk pasien, staf dan orang lain yang relevan harus
dilakukan dalam semua situasi.(Brunero & Lamont, 2017)

2.1. Prinsip otonomi
otonomi mengacu pada kemampuan seseorang untuk membuat keputusan sesuai dengan
nilai pribadi mereka. Dalam praktik keperawatan, mendapatkan informed consent
adalah dasar untuk menghormati otonomi pasien. Penggunaan restrain fisik tanpa izin
pasien bertentangan dengan prinsip otonomi karena melanggar kebebasan pasien,
kecuali jika perawat dapat menjelaskan alasannya kepada pasien sebelum melakukan
praktik tersebut. Dalam masyarakat modern, otonomi individu memiliki nilai yang
signifikan. Jadi, dalam keadaan apa pun, otonomi pasien harus dipatuhi dan tidak
diabaikan, bahkan untuk pasien dengan kekurangan fungsi mental.
Selain itu, pasien psikiatri harus dirawat di lingkungan terbatas minimum dan dengan
perawatan paling tidak sukarela yang diberikan kebutuhan kesehatan mereka dan
keselamatan orang lain [7]. Rupanya, pasien dengan gangguan serius biasanya memiliki
perilaku agresif tetapi menolak cara intervensi medis. Memang, hanya setelah intervensi
yang tepat dapat agresi dikurangi secara efektif. Namun, dalam hal keamanan,
implementasi restrain fisik gua-rants kepentingan kebanyakan orang dan mencegah staf,
pengekang penerima, dan pasien lain dari kekerasan, tetapi memperoleh dikirim melalui
perawatan sukarela sulit dan harus ditemani oleh berarti tentang memastikan

We advocate for policies at the federal, state, and other organizational levels that
will protect individuals from needless trauma associated with seclusion and restraint
use, while supporting both individual and staff safety.
We take responsibility for providing ongoing opportunities for professional growth
and learning for the psychiatric-mental health nurse whose treatment approach
promotes individual safety, as well as autonomy and a sense of personal control.
We promulgate professional standards that apply to all populations and in all
settings where behavioral emergencies occur and that provide the framework for
quality care for all individuals whose behaviors constitute a risk for safety to
themselves or others.
We advocate and support evidence-based practice through research directed
toward examining the variables associated with the prevention of and safe
management of behavioral emergencies.
We recognize that organizational characteristics have substantial influence on
individual safety and call for shared ownership among leaders to create a work
culture that supports minimal seclusion and restraint use and that will enable the
vision of elimination to be realized.
We articulate the following fundamental principles to guide action on the issue of
seclusion and restraint:
o Individuals have the right to be treated with respect and dignity and in a
safe, humane, culturally sensitive and developmentally appropriate manner
that respects individual choice and maximizes self-determination.
o Seclusion or restraint must never be used for staff convenience or to punish
or coerce individuals.
o Seclusion or restraint must be used for the minimal amount of time
necessary and only to ensure the physical safety of the individual, other
patients or staff members and when less restrictive measures have proven
ineffective.
o Individuals who are restrained mechanically must be afforded maximum
freedom of movement while assuring the physical safety of the individual
and others. The least number of restraint points must be utilized and the
individual must be continuously observed.
o Seclusion and restraint reduction and elimination requires preventative
interventions at both the individual and milieu management levels using
evidence based practice.
o Seclusion and restraint use is influenced by the organizational culture that
develops norms for how persons are treated. Seclusion and restraint
reduction and elimination efforts must include a focus on necessary culture
change.
o Effective administrative and clinical structures and processes must be in
place to prevent behavioral emergencies and to support the implementation
of alternatives.
o Hospital and behavioral healthcare organizations and their nursing
leadership groups must make commitments of adequate professional
staffing levels, staff time and resources to assure that staff are adequately
trained and currently competent to perform treatment processes, milieu
management, de-escalation techniques and seclusion or restraint.
o Oversight of seclusion and restraint must be an integral part of an
organization’s performance improvement effort and these data must be
open for inspection by internal and external regulatory agencies. Reporting
requirements must be based on a common definition of seclusion and
restraint. Specific data requirements must be consistent across regulatory
agencies.
o Movement toward future elimination of seclusion and restraint requires
instituting and supporting less intrusive, preventative, and evidence-based
interventions in behavioral emergencies that aid in minimizing aggression
while promoting safety.
