AFFIDAVIT OF LEGITIMATION

WE, the Spouses RIZALINO LLOVERAS and DESIREE BAGRO,

both of legal age, Filipinos, and residents of Brgy. 37 Reclamation Area,
Tacloban City, under oath declare as follows:

1. That we are the parents of RIZALINO BAGRO, JR. who was born on
June 24, 1993 in Tacloban City;

2. That when RIZALINO BAGRO, JR. was born, we were not yet legally
married although there was no impediment whatsoever for us to
contract marriage, both of us being capacitated to do so;

3. That RIZALINO BAGRO, JR. was born out of wedlock or without the
benefit of marriage, thus was registered at the Civil Registrar of
Tacloban City as an illegitimate child using only the mother’s family
name “BAGRO”;

4. That on August 24, 1993, we were married at the Our Mother of

Perpetual Help Parish (Redemptorist) as evidenced by a copy of our
Certificate of Marriage issued by the church;

5. That as a consequence of our subsequent marriage, RIZALINO

BAGRO, JR. has now become legitimated, he being our natural child;

6. That it is now our desire to change the status of our child RIZALINO
BAGRO, JR. from being an illegitimate to a legitimated child so as to
afford him all the rights being enjoyed by a legitimate child;

7. That we have executed this affidavit for purposes of legitimation of our

child RIZALINO BAGRO, JR. and as a requirement of the Local Civil
Registrar of Tacloban City.
