AFFIDAVIT OF DISCREPANCY

been duly sworn to in accordance with law, do hereby depose and state THAT:

2. When I secured a copy of my Certificate of Live Birth from the

National Statistics Office, Manila, and after scrutinizing the same, I
discovered that gender indicated MALE instead of FEMALE as
recorded in my Philippine Passport as well as in my Marriage
Certificate. Hence, the discrepancy;

3. I am executing this affidavit to attest to the fact that my true and real
gender is FEMALE, as recorded in my Philippine Passport as well as in
my Certificate of Marriage.

4. Finally, I am executing this Affidavit to attest to the truth of the

foregoing statements and for all legal intents and purposes it may
serve.
