I, TESS EMILY D. GUEVARRA, of legal age, married, Filipino and resident of
59 San Pedro Bautista St., Bgy. Damayan, Quezon City after having been duly
sworn to in accordance with law, depose and say:

1. That I am not gainfully employed at present nor I am deriving any income

from any source;

2. That I have three children living with me and my husband is earning only
as a tricycle Driver;

3. That I have no real property and we are only renting our place of dwelling;

4. That I am executing this affidavit to attest to the veracity of the foregoing to

state my indigency for purposes of prosecuting as pauper litigant;
