school environment, SAS Board Policy requires certain health records for every
registered student. According to our records your child has not met SAS Medical
Requirements in the following:

Please refer to the following deadlines for completion:

Monday, September 10th- Non-compliant students WILL NOT participate in

activities such as CISSA, HS Sports, ASA, and off-campus school trips until
medical requirements are met.

Monday, October 8th- Non-complaint students WILL NOT be allowed to

attend classes until all records are received and in order. If you have any question
as to your child’s status please contact your child’s school nurse directly.
