AFFIDAVIT

I, Debendra Kumar Mohanty, aged about 43 years, S/o: Sadashiba Mohanty,

Resident of Balabhadra Lane, P.S: Puri Town, dist: Puri do hereby solemnly
affirm and state as follows:

1) That, I am a P.D.S. retailer vide retailer code No. 2615P115, having my

shop at Puri Municipality Market, Puri as shop No.127.
2) That, in letter No.7783, dtd.24.09.2019 of Puri Municipality, Puri, has
given notice for renovation of said market complex.
3) That, now I am continuing temporarily the said P.D.S. retailing shop in
the house of my relative elder brother namely Rabinarayan Das, S/o:
Madhabananda Das, at Balabhadra Lane, P.S: Puri Town, Dist: Puri
under mouza: Daitapada Sahi, Khata No.415, Plot No.246.
 4) That I swear this affidavit to be produced before the concerned
authority for declaration said facts.
5) That the facts stated above are true to the best of knowledge and
belief.
