AFFIDAVIT

I, XXXXXXX , of legal age, an Indian national, and a resident of

XXXXXXX, after having been sworn to in accordance with law do hereby depose
and state that:

1. I am an Indian national currently working in the Philippines.

2. I have a child XXXXXX with my wife XXXXXXX.

3. We are currently applying for the issuance of a passport for our child.

4. However, because I am currently working in the Philippines, I cannot

personally attend to the processing of said passport.

5. Thus, my wife will be the one who will be processing the passport for our
said child.

6. As such, As such, I am executing this affidavit to attest to the foregoing

facts and to support our application for our child’s passport.
