We at maintenance department would like to request to implement again the request

for repair form before repairing any machineries, equipment and tools. So we can have a
proper scheduling on preventive maintenance and we can for see who must be responsible for
the breakdown of any machineries, tools and equipment. We want that all departments must
comply so we can have a document and proof with all our repaired machineries, tools and
equipments. I have attached the sample draft of the request for repair form for your reference.
