EQUIPMENT LEASING CONTRACT

1. The client/customer hereby confirms and accepts that the tools/machines and accessories issued and delivered are in
good working condition, free from any damage and complete as required. To ensure that the tools/machine and
accessories are in good condition and complete as required, the client/customer or his/her representative together with
the UNITEC PIPE MFG. CORP. personnel/representative shall inspect the tools/machine and accessories upon
delivery and pull out. To this end, the customer shall hold UNITEC PIPE MFG. CORP. and its personnel free and
harmless from any cause whatsoever and in all instances while the tools and accessories are in their possession.

2. The tools/machines and accessories indicated herein requires a bond/deposit of the total amount indicated below. The
tools/machine and accessories shall be released/delivered to client/customer upon receipt of cash or check payment and
after confirmation of UNITEC PIPE MFG. CORP technical engineer or authorized personnel.

3. The tools/machine and accessories describe are to be used specifically for the project and returned as indicated below.

4. The client/customer fully understand that the tools and accessories shall be used only within the premises of the project
stated herein. Otherwise, the client/customer shall make a written request to UNITEC PIPE MFG. CORP. stating the
location where the tools and accessories shall be used. UNITEC PIPE MFG. CORP. shall have the full sole authority
to approve or disapprove such request. If the tools/machine and accessories are not on their intended location on the
agreed date of pull-out nor returned on the date stated above, shall the said tools/machines and accessories be
considered lost. The client/customer bond/deposit amount will be charge as payment for the lost or unreturned tools
and accessories.

5. If the tools/machine and accessories are to be used for an additional period of time, the client/customer shall inform
UNITEC PIPE MFG. CORP. in writing of such request stating the intended date of return, but which request shall be
subject to approval of UNITEC PIPE MFG. CORP. Otherwise, UNITEC PIPE MFG. CORP. will withdraw the
tools and accessories from their intended location on the date agreed upon.

6. The tools/machines listed above has a daily rental amount of One hundred ninety pesos per day (Ph190/day)
starting in the date of delivery of tools/machines which was received and accepted by client/client’s representative or
by pick-up of client/client’s representative until the date of its returned or pull-out of the tools/machines accepted by
UNITEC PIPE MFG. CORP. authorized personnel/representative. The rental amount will be deducted to the
bond/deposit amount of the machine.

7. The client/customer hereby agrees to return the tools/machine and accessories on the agreed date stated above. The
client/customer shall present copies of duly accomplished “Equipment Leasing Contract” and other corresponding
documents pertaining in the delivery of tools/machine as reference.

8. The returned tools/machine and accessories are subject for inspection of UNITEC PIPE MFG. CORP. authorized
representative. Any loss or damage in the tools/machine and accessories caused by client/customer’s negligence will
be charge accordingly depending on the extent of damage or loss of machine parts. The charged amount for the
damage or loss of machine parts will be deducted on the bond/deposit amount of the tools/machine.

9. The bond/deposit shall be returned through check payment within fourteen (14) working days and shall be issued to the
registered client/customer name indicated above.

10. This agreement shall take effect upon signing of both parties on the space provided below.
