I, Naresh Pun, son of Mr. Sun Bahadur Pun and Mrs.Tel Maya Pun, resident of Shikha
V.D.C. Ward No:-1, Myagdi changed into Ghara VDC Ward no:-4, Myagdi on 2063
B.S.(2006 A.D) as per the decision of District Development Committee, Myagdi and now,
Ghara Villiage Development Committee (V.D.C) has been reformed Annapurna Rural
Municipality as per the decision of the Ministry of Federal and Local Development,
Government of Nepal dated 2073/11/27 B.S (March 10, 2017 A.D) and that previous
Ghara V.D.C Ward No:-4 Myagdi, Nepal is now Annapurna Rural Municipality Ward
No:-6, Myagdi Nepal, do hereby solemnly affirm and declare as under:

1. That I personally completed the attached form presented by Immigration of Poland

Service, New Delhi.

2. Information provided in the attached forms is true and correct to the best of my
knowledge.

3. All certificates presented including academic and financial documents and surcharge
bank statement is true.

4. I am fully aware of the Immigration Poland student visa regulations and in any case
if information or documents submitted are fraudulent or tempered within any way, I
shall be personally liable for legal action as determined by court.
