Martin Wilson, 62, was admitted to Lyell McEwin Hospital 10 days
ago after a suicide attempt involving an overdose of Mogodol. The
patient will be discharged in three days time (18/9/08).
History of bronchitis
Heavy smoker
Recently retired. Lives with wife, Joan, in housing trust maisonette
in Elizabeth. 3 adult daughters and 5 grandchildren. Depression
related to gambling addiction – he has lost a lot of money and is
in debt. His wife and family previously unaware of addiction –
angry but also upset about his suicide attempt.
He is very remorseful and ashamed
Wants to overcome addiction
Weak and depressed. Anti-depressants prescribed – Lovan 200g
BP 130/95
BMI 19 – underweight
Poor appetite
Physically unfit

Writing Task

Using the information in the notes, write a letter to the social worker, Ms Jennifer Adams, at the
Elizabeth Community Health Centre requesting follow-up care. Stress that Mr Wilson’s case needs
urgent attention.

In your answer:

• expand the relevant case notes into complete sentences

• do not use note form
• use letter format

The body of the letter should be approximately 180-200 words.
