Registered Marriage Contract of Parents – to determine the legitimacy of the child (photocopy certified
by LCR, present the original)

Any 2 proof of birth (NOTE: The documents must show the correct name of the child, place and date of
brith, name of parents. Submit 1 photocopy certified by the issuing agency or present the original)

o Baptismal Certificate / Certificate of Akeca

o School Record (Form 137E)
o Medical Record/Immunization Record with Dr. or Midwife’s signature over printed
name & designation
o Voter Certification
o PhilHealth Membership Data Record
o SSS Membership Data Record (parents/owner of document)
o Insurance Policy

Registered Marriage Contract of Applicant (18 years & above), if applicable

Valid ID with signature

10 days mandatory posting

Official Receipt of authorized fees

Personal appearance of father/mother to sign the legal instrument: acknowledgment/admission
