AFFIDAVIT

That I, MARIA LOURDES I. AGSALUD, of legal age, widow, and resident of

Nancayasan, Urdaneta City, Pangasinan, after having been duly sworn to in accordance with law
hereby depose and say:

1. That I am the owner of that certain parcel of land situated at Camanang, Urdaneta
City, Pangasinan;

2. That my son, PETER JASON I. AGSALUD, is the owner/proprietor of Shapes and

Angles Builder is occupying a space where he is prosecuting his business at the above
described parcel of land;

3. That Peter Jason I. Agsalud is not paying rental for his possession and occupancy in
the said parcel of land as he is my one of my children;

4. That I am executing this Affidavit to attest to the truth of the facts herein above
stated.
