1. Safety Department Documents were verified like
A. Safety Audit Report, Safety Report
B. Form-29 Incident Report,
C. Onsite Emergency Plan
D. Mock Drill Report
E. Safety Audit Compliance Status
F. Form-37
G. Form-11

2. Monthly Safety Report to be Submitted to DISH

3. Training of workers as per Sect-111A for Safety Training for Plant Safety
and Emergency Preparedness as per Factory Act by 3rd party DISH
Approved Trainer or Internal Faculty approved by DISH
4. Confined space Work certified by DISH approved Competent Persons or
KRIBHCO Engineer to be approved by DISH as per Section 36 as per
factory Act.
5 Load Test Certificate for Fork lift Truck (Form-10) to be submitted/ shown
6 Plant Stability certificates with Load Bearing capacity to be submitted
7 Drawing of individual Plant Layout with detail of plant and equipment
8. Test Certificates for Lifting Tools/ Tackles (Form-10), Pressure Vessels
(Form 11) to be shown

Competent Person D.K Engineers competence certificate to be

shown/submitted.
9. Ammonia Cooling Tower Chlorination Room:
1. adequate number of Chlorine Sensors not available
2. Scrubber System with Blower Facility not available
3. Covering Hood for Chlorine Tonner
4. Corroded pipeline in Chlorination area

10 Mock Drill was conducted at Ammonia Cooling Tower Chlorination Area

at 12: 40 Hours for Chlorine leakage at Chlorine tonner Valve in the
presence of Jt. Director and Dy. Director Industrial Safety and Health.

up Report of employees and Workers, Male Female employee Detail etc.
