AFFIDAVIT

I, John Glenn L. Abela, of legal age, married, Filipino and residing at 032-B Sitio Camp
Dangwa, Alapang, La Trinidad, Benguet, after having been duly sworn in accordance with law,
do hereby depose and state that:

1. That I am the duly authorized and designated representative of CJ Oliva

Construction , with office address at Sitio Maria Basa, Pacdal, Baguio City;
2. That I am granted full power and authority to do, execute and perform any and all
acts necessary and/or to represent the CJ Oliva Construction in the bidding for
Construction of Senior Citizens’ Building (Phase II) Dalicno of the Benguet
Provincial Engineering Office ,
3. That CJ Oliva Construction is not “blacklisted” or barred from bidding by the
Government or any of its agencies, offices, corporations or local governments,
including non-inclusion in the Consolidated Blacklisted of Contractors Report issued
by the Government Procurement Policy Board;
4. That each of the documents submitted in satisfaction of the bidding requirements is
an authentic copy of the original, complete, and all statements and information
provided therein are true and correct;
5. That we hereby authorize the Benguet Provincial Engineering Office or its
authorized representative/s to verify the statements, documents and information
submitted herewith to substantiate our eligibility to participate in the bidding for the
Construction of Senior Citizens’ Building (Phase II);
6. That I am not related to the Head of the Procuring Entity, members of the Bids and
Awards Committee (BAC), the Technical Working Group, and the BAC Secretariat,
the head of the Project Management Office or the end-user unit, and the project
consultants by consanguinity or affinity up to the third civil degree;
7. That CJ Oliva Construction shall abide with existing labor laws and standards,
rules and regulations of the Department of Labor and Employment in connection
with the implementation of the project;
8. That we likewise attest in connection with this bidding of Construction of Senior
Citizens’ Building (Phase II) of the Benguet PEO, that:
a. We have taken steps to carefully examine all of the bidding documents;
b. We acknowledge all conditions, local or otherwise, affecting the
implementation of the contract;
c. We have made an estimate of the facilities available and needed for the
contract to be bid, if any; and
d. We have requested for clarification on all vague provisions of the bidding
documents from the BAC and have received a copy of the Supplemental Bid
Bulletin issued by the BAC in response to said queries.
 9. CJ Oliva Construction is authorized to engage in business which is the subject of
this bidding of Construction of Senior Citizens’ Building (Phase II);
10. That I am making this statement as part of the eligibility requirement for the bidding
of the Construction of Senior Citizens’ Building (Phase II)
