I, AL CHUA, Filipino, of legal age, married and with postal address of No. 1. Central Avenue.
New Era, Quezon City, Philippines, after having been sworn in accordance with law, depose and state that:

1. On 13 November 2019, at a meeting of the Church Council, I was nominated and unanimously
elected to the office Executive Minister or Presiding Elder and as such, I became the corporation sole and
has been exercising the powers and authority of the office;

2. Hence, I shall function and exercise my duties, responsibilities, rights and privileges as
corporation sole and Executive Minister of THE VOICE OF AL AND THE DISCIPLES OF ETERNAL
REALIZATION (THE VADER);

3. I execute this affidavit to attest to the truth of the foregoing for the purpose of registering these
facts with the Securities and Exchange Commission and other concerned government agencies, and for
whatever other legal purpose it may serve.
