Understanding Mooring Incidents

Mooring incidents have occurred throughout


the history of the shipping industry while
using mooring equipment such as windlasses,
winches, ropes and wires. It may be our
familiarity with this equipment that helps
create complacency in the recognition of risk
associated with mooring activities.
Increased attention to crew safety awareness,
activity planning and good seamanship will help prevent incidents during
mooring activities.

Focusing on Mooring Incidents

The safety theme for the fourth quarter of 2014 is


prevention of mooring incidents.

Industry research1 has shown that human error is the


largest cause of mooring accidents, reinforcing the
need for thorough and frequent crew training. Getting
caught in the bight of a rope and snap-back impacts
are two hazards that are preventable.

Effective hazard
Poorly arranged, marked and maintained mooring areas
and equipment can also contribute to both personal
markings
injuries and equipment damage.

Reflective Learning
During the focus on this theme, we recommend that you incorporate a practice
known as “reflective learning,” which has been shown to dramatically increase a
small group’s capacity to absorb information. In this process, a group discuss
an incident relevant to their work environment; aided by engaging materials
(video, slideshow or both); and use questions that personalise learning. The
interaction within the group generates empathy and a commitment to action.
