APPLICATION FOR PROBATION

ACCUSED, assisted by comes to this Honorable Court, and respectfully

alleges that:

1. On August 19, 2019, the Honorable Court rendered a Decision finding

the accused guilty beyond reasonable doubt of the crime of Falsicifaction of Public
Document and sentenced her to suffer a penalty of imprisonment of Two (2)
Years and Four (4) months prision correccional minimum in its maximum period
to Three (3) Years and Six (6) months and Twenty (20) days of prision correccional
medium in its medium period and to pay a fine of Three Thousand Pesos
(P3,000.00) with all the accessory penalties therefor;

2. The accused learned the said Decision on the same date;

3. The penalty imposed upon the accused falls within the coverage of the
Probation Law;

4. That the accused is a first time offender and she desires to avail of
the benefits of the Probation Law;

5. That she has not been sentenced to serve a maximum term of

imprisonment of more than six (6) years;

6. That she has not been convicted of any offense against the security
of the state nor convicted by final judgment of offense punished by
imprisonment of not less than one (1) month and one (1) day and/or a fine of
not less than P200.00;

7. That she has not previously undergone probation under the

provisions of P.D. 968, as amended.

 PRAYER

WHEREFORE, it is respectfully prayed of this Honorable Court to

suspend the execution of the sentence imposed upon the accused and place her
on probation for such period and such terms and conditions as it may deem best.

Accused further prays for such other relies and remedies just and equitable
in the premises.
