CANCELLED Dictation If physician request to “cancel” or “delete” this dictation, pend to customer: HSC_Cancelled Dictation.

Aging Grid Definition Standard
Physicians may enter MRN versus Acct #, which results in Person, Unknown. Perform search by MRN entered by
physician to identify correct patient (pad zeros as necessary).
Visit ID Selection Tips Do NOT change Visit to a Temp Visit #; leave Visit ID as entered by the physician.
Header Info/Formatting Operative Reports have Mandatory Headings. See MTI for detailed information.
DISCHARGE DATE: If the ADT does not contain a Discharge Date, and the physician dictates one, enter this date in the
Date of Discharge: Procedure Date field. If the physician does not dictate one, leave the Procedure Date field blank.

All physician speaker codes have an alpha-numeric ID. The leading alpha characters correspond to the Facility
Physician Codes and BE Codes (Business Entity) as noted below. Example: Griffin, Glenn DO (DO1069) – Physician on staff at Corpus Christi.
