1. Letter of Appointment (Using company letter head as sample attached) - 2 Originals


2. Letter of Appointment (Using Lampiran A4 form as sample attached) - 3 Copies.
3. Business Registration (form 9 and Form 49) - 2 Copies.
4. Photocopy of I/C (Person who signed the appointment letter) - 2 Copies
5. Utilities bill either electricity or water - 1 copy
