NOMINAL DURATION: 40 Hours

MODULE DESCRIPTION: This module aims to provide the trainees with the knowledge,
skills and right attitudes required to collect, summarize, maintain and use data to improve
community health related activities.

MODULE CONTENTS:

LO1. Plan and prepare the necessary resources for data collection

1.1. Introduction

1.2. Definition of terms used in data collection

1.3. Preparing questionnaire and making available

1.4. Pre-testing, modifying and amending questionnaire

1.5. Data collection equipment/material

1.6. Informing the community about data collection date and time

1.7. Inviting community leaders to support data collection process

2.3. Classifying and sorting out the collected information

2.3.1. Disease surveillance reporting formats

ASSESSMENT CRITERIA:

LO1. Plan and prepare the necessary resources for data collection

 Questionnaire is prepared and made available

 Questionnaire is pre-tested, modified and amended

 Necessary personnel are trained on data collection procedures

 The necessary equipment/material are identified to execute data collection

 Member of community is informed about data collection dates and time

 Community leaders are invited to support data collection process

LO2. Interpret and utilize necessary health data

 Basic statistical concepts and procedures are provided

 Necessary health data is collected

 Information collected is classified or sorted out on the basis of a clear


understanding of the purpose for maintaining the database system.

 Steps are taken to maintain confidentiality according to prescribed procedures.

 Vital events are continuously and consistently collected and updated timely

 Data are prepared and utilized according to prescribed procedures and guidelines

LO3. Prepare and submit reports

 Reports are prepared using standard reporting formats

 Reports are submitted to health center and/ or Woreda health office

 Updates and reportable diseases are communicated to the Woreda health office or
health centre according to prescribed procedures and guidelines.

LO4. Use intervention measures

 Discussions are made with key stakeholders regarding the health problem, its
causes and appropriate interventions or solutions

 Briefing materials are provided throughout the consultation process to identify and
clarify issues of interest/concern to stakeholders and own organization

 Feedback is provided to the team leader or work team on the results of the
consultation process

 Positive contributions are made to activities that develop an understanding of the


factors contributing to the health problem of the community

 Further information and data are collected when needed for better interventions
