Application Checklist:

1. Registered Pharmacist Certificate Front and Back.

2. Renewal Passbook with latest Validity page.
3. Diploma/Degree/Pharm D/ M Pharm Original Certificate.
4. Indian Passport (Immigration Stamping pages also if stamped)
5. Visa to the Country (If Received)
6. Employment details along with documents if working in India or Abroad.
7. Request Letter to Registrar for Good Standing Certificate.
(Should consist details like present permanent Residential and working
status and Name, address of Examining authority where GSC to send,
purpose of GSC.)
8. Recently taken clear color passport size photo with white back ground.
9. Signature
10.Should contact Registrar either through Video call or else should visit the
office personally.
11.Postal Charges: Demand Draft in favour of “DHL EXPRESS INDIA
PRIVATE LTD” payable at Vijayawada” for Rs……. At SI.No.III
