Meeting Minutes
June 6, 2019

I. Call to Order
The meeting was called to order by Jacqueline A. Salvador, Grade Chairman, in the library at 1
o’clock in the afternoon.

III. Agenda
A. Result of Monitoring
Some teachers are underload and to make the teaching load of teachers clear, the following
must be observed:
Advisorship = 1 Load
Coordinatorship (regardless of number) = 1 Load
B. Teaching Load of Floating Teachers
Floating teachers must have 300 minutes of teaching load.
C. Attendance
Teachers must record in the log book in logging in and out.
For AM shifters, time in is 5:45 and time out is 12:15
For PM shifters, time in is 11:45 and time out is 6:15
D. Classroom Inventory
Jot down the facilities only that was given or provided by the school. Do not include personal
things.
E. Lesson Planning
The format of the lesson plan in Araling Panlipunan and Edukasyon sa Pagpapakatao are as
follows:
Short-sized paper Title: 12 font size
Body: 11 font size Tahoma font style
F. Independence Day
There will be a display of Philippine flag in the commemoration of the Independence Day.
The pupils are requested to write a reflection about Independence Day.
G. Transferee
The transferees must be given right away a request form for their SF 137.
H. NLS/Drop
Pupils, who reached 40 absences, can be a candidate for NLS.
If a pupil has been dropped, make sure to reflect the last day of attendance in the SF 9 and SF
10.
I. Checking of Forms
The assigned checkers for the following levels are the following:
Kindergarten - Ma’am Mercedes V. Paraiso
Grade 1 and 2 – Ma’am Patricia Delos Reyes
Grade 3 and 4 – Ma’am Marissa B. Nadal
Grade 5 and 6 – Ma’am Daisy B. Angeles

 The general assembly was scheduled on June 13, 2019 at 4:00 pm.

IV. Adjournment
The meeting was adjourned by Ma’am Jacqueline A. Salvador at 2:40 pm.
